Kidney Involvement in HNF1B

HNF1B is the subtype where diabetes is not the whole diagnosis, and it deserves separate attention.

HNF1B-MODY is associated with renal anomalies, often cystic kidney disease. The important distinction is that the kidney involvement is caused directly by the gene rather than by years of high blood glucose.

That difference matters practically in three ways.

Timing. In ordinary diabetes, kidney damage follows sustained high glucose over years, so screening starts later and looks for gradual change. In HNF1B, structural kidney abnormalities can be present from the start, including before diabetes appears.

What is monitored. Alongside the usual urine albumin and filtration-rate tests, imaging of the kidneys is relevant because the abnormalities are structural. Someone with unexplained kidney cysts and diabetes should have this combination taken seriously as a pattern rather than a coincidence.

Who looks after you. This subtype often needs a kidney specialist alongside diabetes care, rather than diabetes care alone.

It is also worth knowing in the other direction. If you have been found to have kidney cysts or structural kidney abnormalities and you also have young-onset diabetes with a family history, HNF1B is worth raising as a possibility — it is a recognisable combination.

Because HNF1B is inherited like the other subtypes, relatives may have kidney findings as well as, or instead of, diabetes.
