Why Insulin Can Never Be Skipped

Reviewed by CDE Mehandi Sharma · Updated

In Type 2 diabetes, a missed tablet is usually a minor setback. In Type 1 it is a different matter entirely, and it is worth being blunt about why.

Type 1 means the body produces essentially no insulin of its own. Injected insulin is not topping up a supply — it is the entire supply. Almost everyone with Type 1 needs it every single day, without exception.

Skip it and glucose cannot get into your cells. Your body starts breaking down fat for fuel instead, which produces ketones, and ketones building up turn the blood acidic. That is diabetic ketoacidosis, and missing insulin doses is one of its two most common causes, alongside illness.

This can develop within hours, not days.

It is also why insulin must continue when you are unwell and not eating. That feels wrong — no food, so no insulin — and it is the single most dangerous mistake in Type 1. Illness raises glucose even without food, so the need goes up, not down.

Doses may need adjusting for illness, activity or a missed meal. Adjusting is a conversation with your doctor. Skipping is not an option.
